Step-by-Step Guide: Prescribing Pathway for Overdue Medication Reviews


1. Accessing the Prescribing Pathway System

  • Click on the prescribing pathway button in your toolbar.
  • If it’s not on your toolbar, request to have it added.
  • The system may also remind you to use it if you forget.

2. Handle Overdue Medications

Example 1: Medication Review Booked

  • Scenario: A patient’s medication review is booked, but their review date has passed.
  • Action:
    • Click on the button for this case (e.g., "Issue One More Script").
    • Select "Issue One More Script" if necessary.
    • Move the review date to today.
    • Click "Record Notes" to add codes and free text explaining the action taken.
  • Adjust Review Date:
    • Click on the medication, then the number seven icon.
    • Change the review date to today.
    • This will allow issuing the medication without reauthorizing it.

Example 2: Annual Review Invite Pending

  • Scenario: Patient has not had their annual review invite yet and their medication is overdue.
  • Action:
    • Click on the button (e.g., "LTC Blue").
    • Select "Issue One More Script" and "Send Invite".
    • If you do not want to send the invite immediately, record it in the notes and task reception or admin to send the invite.
  • Notes: This ensures that the patient receives an invite for their annual review.

Example 3: No Response to Invites

  • Scenario: Patient has not responded to review invites and their medication is overdue.
  • Action:
    • Click on "Record and Send SMS".
    • Send an SMS informing the patient of their overdue review and that if they do not contact you, only two weeks of medication may be supplied at a time.
    • Message Example:
      Dear [Patient Name], You are overdue for a review of your long-term conditions medication. Please contact us to arrange a review before your next prescription is due. If you do not arrange this, we may only be able to supply two weeks of medication at a time.
      
  • Reason: Reducing access to medication encourages patients to book their reviews and meets CQC standards for safety.

3. Review and Follow-Up

  • Ensure that all actions are recorded accurately in the system.
  • Follow up on tasks and invites to ensure timely completion.
  • If the patient does respond and completes their review, update their prescription accordingly.

4. Additional Notes

  • The system is designed to minimise tasks for GPs and pharmacists, speed up processes for patients, and ensure safety.
  • If you encounter issues or are unsure about any steps, contact support for assistance.
