Our template allows you to record that a patient has a Reasonable Adjustments Digital Flag (RADF) and record any specific communication needs a patient has.

The template looks like this:
The left-hand side panel will show any details in the patient's record of any reasonable adjustments they may have coded.
Reasonable Adjustments Essential Coding
All 3 of these must be correctly recorded to ensure the patient has a Reasonable Adjustment Digital Flag (RADF) uploaded to the NHS Spine digital flag records.
Review of Reasonable Adjustments previously added
If you are reviewing an existing record of an RADF, you can select this option to code a review of the patient's reasonable adjustments.
Information & Communication
If the patient has any specific needs you would like to record, you can select that needs have been identified, allowing you to complete the next section. If they have no current needs, you can press ok to save the template.
Specific Information or Communication Accessibility needs
This section will remain greyed out unless you select that you have identified a need in the section above.
Any codes selected in this section will be uploaded to the patients RADF, if you have correctly coded all 3 of the essential coding boxes in the first section.
The final part of this template allows you to record whether the patient needs to be reviewed before sending out any recall monitoring invitations. This helps to ensure the patient will receive communication in a format that they can understand based on their personal needs. If the patient does requires a specific form of communication type, please select yes and add details to the free text box.

Prompts for patients to review
We have added in 'Other work to do' a prompt for clinicians to review patients with certain conditions, to see if they need reasonable adjustments recording.
If you see this in the patient recall overview, right click and select action, and the RADF template will launch for you to complete.
