How do I send and action Specific Medication questionnaires?

Sending Questionnaires|


As well as our general med review questionnaires, we have a number that are specific to particular medications. Currently we have the following in place:

  • Oral Contraception - POP and CHC
  • Anti-Depressants / SSRIs 
  • Epilepsy Monitoring
  • HRT
  • Vaginal Oestrogen HRT


Each of these also links to optional annual reviews that can be included in the recall system. For example, you may have opted to include HRT as a review to be booked and completed as part of the annual review process.


Because of this, there are 3 different stages at which you may want to send these questionnaires to relevant patients...


1) Prior to their main annual review invite - that's because completion of the questionnaire may mean that either no review appointment is needed, or that part of the review is completed. For example, a patient on POP who also needs a Diabetes review may complete the POP questionnaire and so only need a Diabetes review booking. These patients are found in 1 Annual Review Invites / section 1m:


2) At the same time as being invited for their annual review This will exclude patients who have already booked their annual review. These patients are found in 1 Annual Review Invites / section 1n:


3) After their annual review is completed (more applicable if you've not opted to include these particular reviews as part of the annual review process). These invites are found in Prescribing / 2 Admin Actions / 1b:


Each of these invite reports excludes patients who've already completed the relevant questionnaire or review, or who've been sent the questionnaire in the last 10 days or twice in the current QOF year.



Processing Questionnaires


Completed questionnaires come into SystmOne as a task to be actioned. You shouldn't need to manually review each completed questionnaire as reports pick up any essential actions. The questionnaires themselves also direct patients to book appointments where necessary.


The pathway is different for each questionnaire. Answers that need to be reviewed can be seen on the prescribing overview template and there are options on there to tick when a specific review has been completed:



1) Oral Contraception - if questionnaire is completed and the patient isn't identified as being in UKMEC 3 or 4, the Pill check is marked as complete. For patients on Combined Hormonal Contraception, they must also have a BP and weight recorded.


If patients are in UKMEC 3 or 4, based on the questionnaires or existing information in their records they go into reports to review:


The Prescribing Overview template will provide more information about why the patient is in a particular UKMEC category, e.g.:


If questionnaire answers tell us they no longer need the contraception, but have it on repeat, the patients will appear in these reports:



2) Anti-depressant / SSRI

Depending on answers, the questionnaire itself will direct patients to book follow-up. However, all completed questionnaires drop into these reports to review the answers: 


3) Epilepsy

If patient tells us they are suffering seizures and not under secondary care, the questionnaire will ask  the patient to book appointment to review thier Epilespy. They will also appear in these reports to be invited for a review:


4) HRT

Depending on answers, the questionnaire itself will direct patients to book follow-up. 

If answers suggest an appointment is needed, they will appear in the following reports:


Otherwise, they will appear in one of the following reports to review their answers:
