How do I process a completed Asthma Control Questionnaire?

This video shows you how to process a completed Asthma Control Questionnaire. The video is a little outdated in places, though the principles are the same. We've done some updated step-by-step instructions below.




Updated step-by-step instructions


  1. Completed questionnaires will appear as tasks in your system. These should be actioned to file the patient records. The answers do not need to be read, as patients will be included in the appropriate clinical reports for the next step.
  2. The following day, patients will appear in these reports:
  3. The ones in the reports "SEND - Comms..." need nothing more than what's in the report name - sending a message to the patient and adding a code. Patients are in these reports as their answers indicate they need an appointment to review their Asthma.
  4. Patients in the report "1d) RETRIEVE RECORD - Click on Pink / Purple status alert and follow instructions to process ACQ" are likely to be appropriate for a notes-based asthma review, but protocols need to be run to check the answers and generate personalised Asthma Care plans. As the report name suggests, each patient needs to be retrieved and processed individually by clicking on the pink/purple AR status alerts:
  5. Select "PROCESS now" on the window that appears:
  6. The system will determine the next steps based on the responses. There are 3 possible outcomes...
    1. Well Controlled: The system logs the responses and generates a personalised care plan.
    2. Poorly Controlled: A preset message is generated to ask the patient to visit for a review.
    3. Intermediate Control: Suggests a clinician review due to some reported exacerbations or issues.
  7. If the outcome is Well Controlled you will see a window like this: 
  8. For most patients, a personalised Asthma Care Plan with a covering letter will be generated. If the patient has inhalers on repeat that could be used for a MART of AIR treatment regime, you'll see this window:
  9. Don't panic at this point! You may not be familiar with asthma treatment regimes, but our protocol will help make it clear. Pick the option "Review Repeat Medication":
  10. The key information is dose the instructions. There are three types of treatment plans. The inhalers themselves might be the same, but the dose instructions determine the type of treatment plan:
    1. Standard - this would just have information about doses to take each day, e.g. "Take two puffs a day"
    2. MART (Maintenance and Reliever Therapy) - i.e. where the same inhaler is used a set number of puffs daily, but can also be used for extra doses when needed. This is instead of a separate inhaler that's used as a reliever. As well as the daily doses, there's instructions about increasing when needed and a maximum number of doses per day. The example in the image above is for a MART therapy.
    3. AIR (Anti-Inflammatory Reliever) - i.e. an inhaler that is only used when required and not every day, e.g. "AIR. Inhale ONE dose when required for symptom relief. Up to 6 doses at one time. Up to 8-12 doses in 24 hours."
  11. It may take a bit of getting used to but you'll soon learn to recognise the different regimes. On the next screen, you pick the appropriate treatment regime (though if you're still really unsure, there's an option to task someone):
  12. On the New Letter generation window, click "Write Now":
  13. In Word, you can read and sense check the care plan. The covering letter will clarify if a patient needs further appointments. If you're not sending the letter via comms annex Print and send the letter, otherwise, just click "Save Final Version":
  14. Then follow the instructions in this view:
  15. Send the comms annex message, close the view and save the record.


Processing Paper Answers


If you send paper copies of the questionnaire, the first step is the main difference in processing completed answers. The patient will likely have an AR Red status alert. Clicking on that will give an option to enter their answers:

You will then see a version of the questionnaire that you can complete with the patient's answers:


When that's completed, click "Save Final Version" and then the protocol will continue to guide you.
