Even if a patient is under secondary care for diabetes, we'd recommend they're still invited for at least a stage 1 review as secondary care may not always do foot checks and you may not get the data you need from them either.
If you do 2 stage Diabetic reviews, you can opt to exclude patients from a second stage review if they're under secondary care. You'd need to let us know if you want to do that.
To exclude an individual patient from a stage 2 diabetes reviews, there are three requirements that need to be met:
- Your practice needs to have opted into this.
- They need to be coded as "Diabetes care by hospital only", without a more recent code of "Diabetes: practice programme" or "Diabetes: shared care programme"
- They need to have been coded with some kind of "Seen in Diabetic clinic" code or similar since the start of the last financial year. This is to prevent patients getting lost to follow up.
If these criteria are all met, the system won't direct patients to have a second stage review and the Recall Overview will display that they're excluded from the second stage and explain why.
